When a patient’s treatment plan involves dental implants, our surgeon meticulously plans the procedure and studies the patient’s oral CT scan to ensure correct implant placement. Skilled planning is the key to successful implantation, long-lasting results, and the absence of complications. At this stage, the surgeon may encounter a situation where there is insufficient bone for implantation. If the lack of bone tissue is located in the area of the maxillary sinuses in the upper jaw, you will be offered a preparatory procedure for bone augmentation, which is called a sinus lift.

Sinus lift techniques

There are two main techniques for performing a sinus lift procedure:

Closed sinus lift

A special technique of bone augmentation considered the most gentle and safe. To use this technique, a minimum bone height in the area of the planned implantation is required — at least 5–6 mm. In our clinic, we perform the closed sinus lift procedure together with the placement of the dental implant.

Key steps of the closed sinus lift procedure at Masych Clinic:

  • First, the surgeon prepares the bone site for the implant.
  • Next, through the created opening, access to the floor of the maxillary sinus is obtained. The surgeon lifts the Schneiderian membrane (the mucous membrane) upward.
  • The created space is filled with pre-prepared bone graft material.

The main drawback of this method is that it cannot significantly increase the bone height. Therefore, if the available bone is less than 5 mm, this technique will not be effective.

Open sinus lift

The main advantage of the open sinus lift is the ability to effectively increase a significant volume of bone tissue, regardless of its initial height.

If the initial bone height is more than 3 mm, it is possible to perform an open sinus lift with simultaneous implant placement, similar to the closed sinus lift.

Key steps of the open sinus lift procedure at Masych Clinic:

  • First, the surgeon creates access to the maxillary sinuses from the lateral side of the upper jaw.
  • Next, the surgeon carefully detaches the mucous membrane (Schneiderian membrane) from the bone and lifts it upward.
  • In the next step, a special protective membrane is placed under the mucous membrane, and the remaining space is filled with specially prepared bone graft material.
  • The bone window, through which the surgical access was made, is also covered with a protective membrane to promote better healing.
  • When feasible, the surgeon places your chosen implant and then sutures the gums.

The advantage of the open method is that it allows for a significant increase in bone tissue over a large area, sufficient for placing 2–3 implants. If immediate implantation (along with the sinus lift) is not possible, the implant placement is performed after 6–9 months.
