Root canal retreatment in Ivano-Frankivsk
Sometimes, a tooth that has already had root canal treatment can start causing problems again after a while. You might feel pain or discomfort when chewing. Swelling or a fistula may develop on the mucosa, through which inflammatory exudate may be discharged during exacerbation. In some cases, the problem is initially asymptomatic (chronic process), during which a granuloma (bone tissue defect) forms at the apex of the root.
Reasons for complications after initial root canal treatment.
The most common causes of complications following primary root canal treatment are:
- Failure to maintain asepsis and insufficient isolation of the working area (procedure carried out without a rubber dam)
- Insufficient or missing irrigation. Root canal irrigation involves flushing the canals with different solutions and activating them (cavitation effect) using specialized equipment and instruments.
- Poorly sealed or inadequate root canal filling. If the canals contain underfilled areas, voids, or gaps between the filling material and the canal walls, the canal can become reinfected, leading to tooth pain.
- Improper or non-hermetic placement of a coronal restoration. Microgaps in the filling allow saliva and food debris to enter, which can also lead to reinfection of the root canals. Specialists at Masych Clinic always complete primary root canal treatment with the placement of a crown. According to the latest international protocols, crown placement is a mandatory final step of root canal therapy, as a devitalized tooth (i.e., a tooth from which the blood vessels and nerves have been removed from the root canals) loses its nutrition, becomes brittle, and is prone to fracture. A tooth restored with a ceramic crown will last for many years, and reinfection of the root canals due to leakage is completely eliminated.
- If the filling material goes beyond the tip of the tooth root, it can cause inflammation to develop later.
- Lack of a microscope during primary root canal treatment. Performing endodontic therapy without a microscope is essentially working “blind,” without visual control, which significantly reduces the likelihood of a favorable treatment outcome. There is a saying: “we cannot treat what we cannot see.” Anatomical variations of root canals often do not correspond to textbook descriptions; canals may be curved or, more commonly, present as additional branches that cannot be adequately cleaned or medicated through mechanical or chemical treatment.
- Instrument fragment retained in the root canal after initial treatment. If an instrument breaks inside the canal, the clinician’s skill and available equipment may not always allow its removal. Consequently, bacteria may remain around the fragment, potentially causing reinfection and pain over time.
- Step formation or perforation. During primary treatment of a curved root canal, if the clinician fails to follow the canal path and instead deviates into the canal wall, a step is created. If the wall is completely breached, the complication is termed a perforation. These complications may become clinically evident sometime after canal obturation.
